You have the right to request and receive copies of your health records. Requests must be in writing. We may charge a reasonable fee for copies and mailing.
2. Right to Request Amendments
If you believe your PHI is inaccurate or incomplete, you may request an amendment in writing. If denied, we will explain why in writing.
3. Right to an Accounting of Disclosures
You may request a list of certain disclosures made within the past six years, excluding those made for treatment, payment, or operations.
4. Right to Request Restrictions
You may request limitations on the use or disclosure of your PHI. We are not required to agree unless it involves restricting disclosures to a health plan when you’ve paid out-of-pocket in full.
5. Right to Request Confidential Communications
You may request that we communicate with you in a specific way (e.g., by phone, email, or mail), and we will accommodate reasonable requests.
6. Right to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us or the U.S. Department of Health and Human Services (HHS). We will not retaliate for filing a complaint.
